The Intus Care care management software allows provider organizations to track the efficiency of care providers through electronic visit verification (EVV). In addition, Intus Care has developed a machine learning approach for the prediction of hospital readmissions and utilization risks that leverage electronic health record (EHR) data through automation. This automation is paired with a patient-centered assessment to decrease hospitalizations for patients in long term care facilities. Additionally, the Intus Care platform allows for the sharing of this information between relevant stakeholders in a patient’s care continuum to assist timely, data-driven interventions.
